PROFILE-MD IS NOT A TELEMEDICINE APPLICATION.

THE PERSON SPECIFIC INFORMATION CONTAINED WITHIN PROFILEMD IS ENTERED BY THE END USER. THIS INFORMATION IS SUBJECT TO DATA ENTRY ERROR AND MAY BE INCOMPLETE. IT IS NOT A SUBSTITUTE FOR OBTAINING A MEDICAL HISTORY ON A PATIENT. IT IS ESSENTIAL THAT ALL INFORMATION CONTAINED WITHIN PROFILEMD BE CONFIRMED.

ALL GOALS FOR BLOOD GLUCOSE CONTROL, BLOOD PRESSURE, WEIGHT, PEAK FLOWS, LABORATORY VALUES, AND FREQUENCY OF PREVENTATIVE MEDICINE TESTS ARE ENTERED BY THE END USER, AND THEREFORE MAY NOT REPRESENT GOALS THAT WOULD BE CONSIDERED OPTIMAL IN WHEN COMPARED TO UP TO DATE MEDICAL LITERATURE. THE RESPONSIBILITY FOR THE APPROPRIATENESS OF THESE GOALS FALLS TO THE END USER. PLEASE DISCUSS WHAT YOUR GOALS SHOULD BE WITH YOUR PHYSICIAN.

ProfileMD is published independently from any pharmaceutical company, medical device manufacturer, clinic, hospital, research study, or other health care organization. Organizations may distribute ProfileMD as a promotional/educational item. However, the authors of ProfileMD determine all content of this program.
ProfileMD uses a password system to protect the privacy of the information you enter into the database. Failure to accurately record the password will make your information unavailable. We will not be able to unlock the databases without your password (there is no universal password for this program!), which will result in loss of your data if you do not remember your password.
Computer technology may allow others to devise means of recovering/reading your information from ProfileMD without your password. Safeguard your handheld device to prevent this from occurring.
